Service Coordination

Mission: The primary mission of the service coordination profession is to serve residents who need and desire services, while empowering them to remain self-reliant whenever possible and paying particular attention to the preservation of successful and affordable housing and the quality of life for the resident population at large. The American Association of Service Coordinators serves and represents service coordinators nationwide who are involved in addressing the service needs of the residents in their respective communities. The mission of the AASC is to advance the interests of the service coordination profession and provide guidance to members in the sustained operation of service-enhanced housing through leadership, advocacy, networking, education and other services.
 
The service coordinator is a service manager to residents and their families who provide informal counseling, information and referral, plan educational programs, coordinate volunteer opportunities, link with outside service agencies and negotiate affordable services as needed, all in cooperation with the community managers.

Goal: The goal of Service Coordination is to assist elderly residents in their ability to age in place, empowering residents while promoting independence.

Purpose: On-site service coordination is made available to residents of Christopher Homes, Inc. apartments so that persons who are frail/near frail may successfully maintain residency and avoid transition into a nursing home if they have a care plan in place and supervision of that care plan by a service coordinator.

Role of service coordinators: A service coordinator is one who provides general assessments of current functionality, needs assessments, care planning, and who provides linkages to other social services while monitoring the success and outcome of those services. The service coordinator does not provide these services directly.

Client care: In respecting the dignity of the elderly and their choices, services are voluntary. No resident shall be forced into receiving services if they do not want them. At the same time, residents who choose to receive services must actively participate in their care plan, be willing to meet with their service coordinator, and follow through on directives.
